How to Order OIT Nametags

To place an order, please complete the OIT Purchase Request Form. Submit the form via e-mail to oit-itpurch@alaska.edu. Please be sure to provide the following information on your request form:

  1.   Your department's FUND and ORG
  2.   The full name of the employee
  3.   The Departmental Listing
